בכל בית אבות סיעודי ישנו מנהל הרפואי, שעליו חלה האחריות על הטיפול הכוללני בבית האבות, וכן רופא הבית אשר נותן את הטיפול הראשוני למטופלים (את שני התפקידים יכול למלא אדם אחד). על שני בעלי התפקידים להיות בעלי רישיון עיסוק ברפואה בתוקף וכן בעלי ניסיון בתחום הגריאטריה. המנהל הרפואי הוא חלק מהנהלת בית האבות, והוא האחראי להשתתפות הצוות הרב-מקצועי של בית האבות בביקורות ופיקוחים, וכן שיפור ותיקון השירות, במידת הצורך. בנוסף, אחראי המנהל הרפואי לקביעת מועדי פגישות עם משפחות המטופלים.
